Medical Social Worker - Home Health

LIC SW ESSENTIAL JOB FUNCTIONS: 1. Assesses the psychosocial status of clients related to the client's illness, environment and support systems 2. Maintains clinical records on all clients referred to social work services in a timely manner 3. Provides information and referral services for HHCJ clients and families regarding practical and environmental needs 4. Maintains collaborative relationships with HHCJ agency personnel to support quality client/family care 5. Presents community education program and/or in-service dealing with health/hospice issues 6. Organizes and coordinates agency bereavement program, including community workshops, support groups, debriefings 7. Participates in activities, which promote HHCJ 8. Assumes responsibility for personal growth and development 9. Performs other duties as assigned

Required Qualifications

Lic. Socia Worker LCSW Must be We must have a LCSW, HH ASAP 13-26 weeks

Preferred Qualifications
