Speech & Language Pathologist - Skilled

Full time hours. SNF experience. Rehab State License Verification. Optima Experience Preferred. Resume. References (3). Flu Shot (During Flu Season Only). Federal Criminal (within 30 days of start). Elder Justice Act, read (signed upon confirmation). HHS/OIG Search. GSA Search. Two Forms of ID. True 2-Step PPD or Quantiferon (2nd PPD must have been placed within 7-14 days after the first PPD was read). Physical (within 6 months of start, must state "able to work with no restrictions and free from communicable diseases").
